Form6 Terms & Conditions of Platform Use
This document sets out the terms and conditions governing the use of the form6 platform. Please read it carefully before registering or using any of our services.
Effective Date: 1 January 2026. These Terms are governed by the laws of Malaysia.
1. Acceptance of Terms
By accessing, registering for, or using the form6 platform in any way, you are deemed to have read, understood, and agreed to be bound by all terms and conditions set out in this document. If you do not agree with any part of these terms, you are not permitted to use the platform.
These terms constitute a legally binding agreement between you as the user and form6 as the platform operator. Continued use of the platform following any amendment to the terms will be deemed acceptance of the updated terms.
2. Account Creation & Security
To access the full suite of form6 services, you are required to register a valid account. During registration, you must provide accurate, complete, and up-to-date information. Each user is only permitted to hold one registered account. Registering more than one account under the same individual is prohibited and may result in all associated accounts being closed.
You are solely responsible for keeping your password and account login details confidential. form6 will never ask for your password via email, message, or phone call. If you suspect any unauthorised use of your account, please contact our support team immediately at [email protected].
Users must be at least 18 years old to register and use the platform. form6 reserves the right to request age verification at any time, and any account found to have been registered by an underage individual will be closed immediately.
3. User Responsibilities & Prohibited Activities
As a form6 user, you agree to use the platform solely for lawful purposes and in compliance with applicable Malaysian law. You are responsible for all activities carried out through your account.
The following activities are strictly prohibited:
- Using automated software, bots, or scripts to access or interact with the platform
- Attempting to gain unauthorised access to systems, servers, or other users' accounts
- Spreading false, misleading, or unlawful content
- Engaging in any form of fraud, money laundering, or other illegal activity
- Sharing your account or allowing a third party to use it
Violating any of the above prohibitions may result in immediate account suspension or closure without compensation, and form6 reserves the right to report suspicious activity to the relevant authorities.
4. Payments, Deposits & Transactions
All financial transactions on the form6 platform are conducted in Malaysian Ringgit (RM / MYR). Users are responsible for ensuring that all payment details provided are accurate and valid.
Deposits that have been successfully credited to an account are final and cannot be reversed, except under circumstances specified by the platform's policy. form6 is not liable for any losses arising from incorrect payment details provided by the user.
Withdrawals will only be processed to an account or payment instrument that has been verified and registered in the same user's name. form6 reserves the right to delay or reject any withdrawal request that shows signs of suspicious activity pending investigation. Any fees charged by third-party payment processors are the user's responsibility.
Form6 reserves the right to change, add, or discontinue accepted payment methods at any time. Users will be notified of such changes via platform notifications or their registered email.
5. Limitation of Liability & Account Policies

The form6 platform is provided "as is" without any implied or express warranties regarding reliability, fitness for a particular purpose, or accuracy of information. form6 shall not be liable for any direct, indirect, consequential, or punitive losses arising from the use of or inability to use the platform, including service disruptions, technical failures, or unauthorised access by third parties.
Account Suspension & Closure
Form6 reserves the right to suspend or close any account at any time, with or without prior notice, if a user is found to have breached these terms, engaged in illegal activity, provided false information during registration, or abused the system. Where an account is closed through no fault of the user, any valid account balance will be returned in accordance with the platform's standard procedures once the investigation is complete.
6. Intellectual Property, Third-Party Links & Amendments to Terms
Intellectual Property
All content on the form6 platform — including logos, designs, text, graphics, software, and interfaces — is the exclusive property of form6 or its respective licensors. You may not copy, distribute, modify, or use any part of the platform's content for commercial purposes without prior written consent from form6.
Third-Party Links
The form6 platform may contain links to third-party websites or services. These links are provided solely for user convenience. form6 has no control over the content, privacy policies, or practices of any third-party sites and accepts no responsibility for any loss or damage arising from their use.
Amendments to Terms and Governing Law
Form6 reserves the right to amend, update, or replace these terms at any time. Any changes will take effect immediately upon being published on the platform. It is the user's responsibility to review the terms periodically. These terms are governed by and construed in accordance with the laws of Malaysia. Any disputes arising from these terms shall be resolved through the jurisdiction of Malaysian courts.
